Halton & St Helens Primary Care Trust

Nationally Stroke is the third largest cause of death and has a greater disabilityt impact than any othe chronic disease. Halton and St Helens Primary Care Trust have launched the Community Stroke Co-ordination Team to support halton residents to support Halton residents affected by stroke. Our Community Stroke Co-ordinators can receive referrals and enquiries from health and social care professionals and members of the public who have been affected by stroke on 0151 495 5038.
